By submitting this form I agree to abide by the following guidelines regarding my request to place my membership on hold: **
- I understand that the effective hold date will be the above selected date. However, payments scheduled to run in the next 7 days will still be applied to my account.
- I understand that my draft will automatically restart after my hold period.
- I understand that I may place my account on hold only one (1) time per calendar year, for up to three (3) consecutive months.
- I understand that if I cancel during the Hold Period, I must submit a separate Cancellation form and it will be subject to the Cancellation Policy per my Membership Agreement.
- I am aware that I will not continue to build towards Superman strength and fitness while my membership is on hold. All gained powers will start to fade the longer I am out of the box.

Membership Cancellation Form

This form may used to cancel Month to Month or 3 Month Agreements and is subject to the terms agreed up in the “Member Agreement”. 3 month membership cancellations must be submitted at least 7 days prior to the next renewal date. Month to month memberships require a 30 day notice.

- By submitting this form, I am giving Back Alley my written notice to cancel my membership per my membership agreement. Should I choose to rejoin Back Alley after the termination of my membership, I will have to pay at the then current rate.
- I am submitting a 30 day notice of cancellation as required per the membership agreement. I understand that by canceling my membership during a contract period will result in a cancellation fee as agreed upon in the membership agreement. This fee will be applied immediately and my membership will be deactivated at the end of the last paid month prior to cancellation. (Agreement can be viewed at anytime through your Wodify account).
